The above referenced entities and/or persons hereby request consideration for credit, as either a loan or a lease, with Commercial Equipment Finance, Inc. (including its subsidiaries and affiliates and their respective agents, officers and/or employees; collectively, “CEFI”); by agreement and signature of this credit application, the undersigned acknowledges that CEFI will rely on this information provided in connection with this application. The undersigned has no knowledge of any liabilities, contingent or otherwise, not reflected in this application. The decision to grant or deny credit is at the sole discretion of CEFI, and no credit shall be granted unless or until approved in writing by the authorized persons of CEFI. All information in this application and all attachments are correct to the best of my knowledge. I authorize CEFI and/or its lenders, participants and assigns (collectively, “CEFI Successors”) to conduct any credit investigations it deems appropriate, including but not limited to verifying and confirming employment, character, general reputation, and all financial and other information submitted with this application (collectively, the “Credit Information”), as well as obtaining a consumer credit report, for CEFI and CEFI Successors to act on this credit application. I further authorize CEFI and CEFI Successors making continued inquiries, verifications and confirmations about any such Credit Information and obtaining a consumer credit report during the term of any obligations with CEFI and CEFI Successors. As required by law, my identity will be verified. I authorize any person, including, without limitation, all past or present creditors, trade references, financial institutions, banks, suppliers, credit reporting organizations, governmental authorities, or any institution providing credit information (collectively, the “Requested Parties”) to compile, furnish and/or release to CEFI and CEFI Successors any and all necessary financial and credit information, personal and otherwise, and to respond fully to requests for information based on this application when transmitted by electronic or other means. The above permissions and authorizations will apply to Requested Party to whom this application is submitted. For the purposes of any applicable statute pertaining to the privacy of information this clause shall constitute full and sufficient consent for the collection, use and disclosure of information, as required for a credit investigation. I certify that the obligations applied to hereunder are for business, commercial or agricultural purposes and not for personal, family, and household purposes. I certify that authorization has been obtained from those listed above, however have not signed below, to obtain their Credit Information as they are applying to CEFI and CEFI Successor for a credit transaction. I agree that CEFI and CEFI Successors may freely communicate Credit Information, transactional, and other information about me or any stockholder, partner or member of any legal entity identified above, in connection with the administration, servicing and/or management of any credit asset, including releasing credit bureau reports to lenders working with CEFI and who may provide credit facilities relating to this application. Any information that it may have or obtain in response to such credit inquiries and agree that such information, along with the application, shall remain CEFI’s property whether or not the credit application is approved. FAIR CREDIT REPORTING ACT: If your application for business credit is denied, you have the right to a written statement of the specific reasons for the denial. To obtain the statement, email [email protected] within 60 days from the date of decision notification. A written statement of reasons for denial will be sent within 30 days of receiving request.
